Mounjaro (tirzepatide) is highly effective for weight management, but like all GLP-1 medications, it comes with a well-documented side effect profile. Understanding what to expect — and how to manage it — is one of the most important things you can do to stay on treatment and get the best results.

How Common Are Side Effects?

According to the MHRA-approved product information for Mounjaro, gastrointestinal side effects are very common — affecting more than 1 in 10 people. The clinical trial data (SURMOUNT programme) showed that side effects are most pronounced during dose escalation and typically improve as the body adjusts.

The good news: the vast majority of side effects are mild to moderate and transient. Fewer than 5% of participants in the SURMOUNT trials discontinued treatment due to side effects.

The Most Common Side Effects

1. Nausea

Nausea is the most frequently reported side effect, affecting up to 40% of patients during the early months. It tends to peak on days 2–3 after injection and resolve before the next dose. Practical strategies include:

  • Eating smaller, more frequent meals
  • Avoiding fatty, spicy, or rich foods around injection day
  • Staying upright for 30–60 minutes after eating
  • Keeping well hydrated — sipping fluids rather than drinking large volumes at once
  • Ginger tea or ginger biscuits (evidence supports ginger for mild nausea relief)

If nausea is severe or persistent, speak to your prescriber about suitable options.

2. Constipation

GLP-1 receptor agonists slow gastric emptying and gut motility, which commonly causes constipation. This is particularly common at higher doses (10mg+). To manage it:

  • Increase dietary fibre gradually — vegetables, legumes, wholegrains
  • Drink fluids regularly, unless your care team has advised you to limit fluids
  • Ask your pharmacist or prescriber which constipation treatment is suitable for you
  • Gentle physical activity helps stimulate bowel motility

3. Fatigue

Fatigue is commonly reported, particularly after each injection. It is likely multifactorial — reduced caloric intake, the physiological effect of the medication, and disrupted sleep from nausea all contribute. Speak to your care team if fatigue is persistent or severe; they can help assess its cause.

4. Vomiting and Diarrhoea

Less common than nausea and constipation, but they do occur — especially during dose escalation. If vomiting is frequent or prolonged, this poses a risk of dehydration and electrolyte imbalance. If vomiting persists or you cannot keep fluids down, seek medical advice promptly rather than waiting 24 hours.

5. Hair Thinning

Telogen effluvium — a diffuse, temporary hair thinning — affects a subset of patients on rapid weight loss journeys. It is caused by the physiological stress of significant caloric restriction rather than the medication itself. It typically begins 2–4 months into treatment and resolves as the body adjusts. Ensuring adequate protein, zinc, and iron intake is the primary clinical recommendation.

When to Seek Medical Advice

Contact your prescribing team or NHS 111 promptly if you experience:

  • Persistent vomiting, inability to keep fluids down, or signs of dehydration
  • Right-sided abdominal pain, fever or jaundice
  • A new rash or other possible allergic symptoms without breathing difficulties
  • Low blood sugar symptoms if you also take diabetes medicines

Adverse reactions should also be reported to the MHRA via the Yellow Card scheme at yellowcard.mhra.gov.uk.
